• Injury Type – Indicates if the employee was injured while performing normal work duties or not.
• Case # - The case number assigned to this incident.
• Injury Date – The date the injury occurred
• On Premises? – Indicates if the injury occurred on company owned property.
• Injury Time – The time that the injury occurred.
• Time Unknown – If the time of the injury is not known check this box.
• Place of Injury – Describes the place where the injury occurred.
• Body Part Injured – A description of the body part or parts injured.
• Object Causing Injury – A description of the object causing the injury.
• Days Missed From Work – The number of days the employee missed work as a result of this injury.
• Days Work Transferred or Restricted – The number of days the employee worked at an alternate position or in a limited capacity as a result of this injury.
• Was employee treated in an Emergency Room? – Indicates if the employee was treated in an emergency room.
• Time Employee Began Work – The time that the employee began working the shift during which the injury occurred.
• Physician Name – The name of the main doctor that treated the employee.
• Physician Address – The address of the doctor that treated the employee.
• Hospital Name – The name of the hospital where the employee was treated.
• Hospital Address – The address of the hospital where the employee was treated.
• Job Title – The employee’s job title when the injury occurred.
• Category – The OSHA category for this injury.
• Result – The OSHA result for this injury.
• Was employee Hospitalized Overnight as an in-patient? – Indicates if the employee spent a night in the hospital on an in-patient basis.
• Date of Death – In the case of a fatality enter the date the employee died here.
